VA Community Care Network
VA provides supportive medical services to help chronically ill or disabled eligible Veterans of any age remain in their home. These services are referred to as home health care and are considered extended care services prescribed by and under the direction of a VA physician.
Vet-Assist
Through Veteran Home Care's Vet Assist Program, veterans and surviving spouses can receive care before the VA reviews and approves the Pension application and funds arrive
Options Programs
Pennsylvania’s Options Program (also called “Help at Home”) provides aging residents with care services in their homes, such as personal care, home health, and home delivered meals. Assistance is also provided to family caring for a loved one at home, such as adult day care, respite care, and specialized medical transportation assistance. The Options Program begins with a comprehensive assessment of the applicant’s health and physical abilities, as well as their income. The Pennsylvania Consolidated Waiver is designed to help individuals with an intellectual disability, autism or developmental disability to live more independently in their homes and communities and to provide a variety of services that promote community living, including self-directed service models and traditional, agency-based service models.
Community Living Waiver
The Community Living Waiver supports individuals with an intellectual disability, autism or developmental disability to live more independently in their homes and communities through the provision of a variety of services that promote community living, employment, communication, self-direction, choice and control.
Person/Family Directed Support (P/FDS) Waiver
The Pennsylvania Person/Family Directed Support Waiver is designed to help individuals with an intellectual disability, autism, or developmental disability to live more independently in their homes and communities and to provide a variety of services that promote community living, including self-directed service models and traditional, agency-based service models.
