Currently hiring for pediatric shift care. Full time and Per Diem positions available.

Responsibilities: 

• Evaluates the patient and home health environment to ensure Universal Standard Safety Precautions are valued

• Serves as our clients advocate and maintains ongoing communication with Physicians, referral sources, caregivers, and facility management to facilitate coordination of care

• Provides direct supervision of nursing clinical care team members; LPN's / HHA's.

• Implement patient's plan of care to LPN / HHA.

• Complete Initial Assessments, Re-Visit Forms and/or Medical Orders

Qualifications:

• Pennsylvania State Licensed Registered Nurse Certification

• Valid form of Identification

• Mandated computer literacy

• Excellent verbal and written communication skills
