DIRECT CARE WORKER
The Direct Care Worker provides direct care and support to clients in residential and/or day program settings. This position is responsible for helping clients reach their highest levels of independence and safety, while ensuring their quality of life.
Duties & Responsibilities:

Provide direct care and assistance to residents/clients in accordance with individual service plans, program policies and procedures.

Assist in developing and implementing daily activities that promote physical, emotional, intellectual and social growth

Monitor clients’ activities and document progress

Assist clients with personal hygiene and daily living activities

Lead and participate in recreational activities and leisure time activities
-
Monitor and report any changes in clients’ conditions to supervisor

Provide transportation to doctor appointments, community activities, and other related outings

Participate in team meetings, trainings and other educational activities
-
Maintain a safe environment for clients

Comply with all relevant laws, regulations and agency policies
Qualifications:

A valid form of Identification

Mandated computer literacy

Excellent verbal and written communication skills

Ability to develop and maintain a respectful, caring, tolerant, ethical, and empowering relationship with individuals with intellectual and developmental disabilities;

Capability to assist/lift individuals with physical disabilities and/or limited mobility;
