LICENSE PRACTICAL NURSE
The LPN will be responsible for providing direct patient care and will assist in the assessment, planning, implementation, and evaluation of patient care. The LPN will be responsible for administering medications, monitoring vital signs, and providing patient education. The LPN will also collaborate with other members of the healthcare team to develop and implement nursing care plans.
Responsibilities:

Provide direct patient care, including administering medications, monitoring vital signs, and providing patient education

Collaborate with physicians, nurses, and other healthcare team members to develop and implement nursing care plans

Document patient care and progress, including patient assessments, care plans, and discharge plans

Provide education and support to patients and their families

Ensure compliance with state and Agency Policys
Qualifications:

Valid Pennsylvania LPN license

excellent communication and problem-solving skills

ability to work both independently and in a team environment
